The item import spreadsheet is the ideal tool for registering questions in bulk to your checklist. With it, you speed up the creation of questionnaires by filling out the information in a standard template and uploading everything at once to the system.
Preparing the file
When downloading the spreadsheet template from the system, you will notice that it contains guidelines and filled-out examples:
- Row 2: Contains instructions on how to fill it out.
- Rows 3 to 6: Are practical examples of how to fill out the columns.
Attention: Before saving the file for import, it is mandatory to delete the example rows (from row 2 to row 6). Leave only the header (row 1) and your data.
Filling out the columns
To configure your items correctly, understand the function of each column available in the template:
- Position: Defines the hierarchy. Determine whether the item belongs to a first area, subarea, or subsequent areas.
- Area name: The name of the group where the item will be located (e.g., Production, Kitchen, Reception). Items with the same area name will be grouped together.
- Item text: The statement of the question that will be displayed to the evaluator.
- Item type: Select the answer format from the drop-down list (e.g., Text, Yes/No, Selection List).
- Selection list options: Fill out only if the "Item type" is "Selection list". Otherwise, leave it blank.
Tip: The second tab of the spreadsheet (Scales List) is for reference purposes only to view available item types. It is not necessary to delete it to perform the import.
Selection List Configurations
For items of the "Selection List" or "Multiple Choice" type, use specific formatting in the Selection list options column:
- Separate options: Use a semicolon (;) to separate the alternatives.
- Define weights: Use two vertical bars (||) after the option name to assign a percentage weight.
Example of filling in the cell:
Forklift||10;Packer||20
Note: For multiple-choice items, defining how the grade calculation will be made (sum, average, etc.) must be done directly in the checklist configuration within the system after the import.
Weight, Mandatory Status, and Add-ons
The other columns allow you to refine the behavior of the item during application:
- Additional scales (N.A. / Medal): Define with "Yes" or "No" whether the user will be able to use the "Not Applicable" or "Medal" (positive highlight) options.
- Item weight: Numerical value that the item will have in the final grade calculation.
- Add-ons (Comment, Action Plan, Attachment): Define with "Yes" or "No" to enable these resources for the user.
- Mandatory item: If marked as "Yes", the checklist cannot be finalized without answering this item.
- Item tip: Guidance text that appears when clicking on a help icon during the inspection.
About images in the Tip: If you insert a link in the "Item tip" column, the system will show the clickable link to the user. To display an image or video directly (without a link), it is necessary to configure the item through the system after the import.

1. How to separate options in a selection list?
The options must be separated exclusively by a semicolon (;), with no additional spaces between the separator and the text.
2. How to define the weight of each item in the selection list within the spreadsheet?
Include the symbol || followed by the numerical value right after the option name. Example: Option A||10;Option B||0.
3. Where do I configure the calculation for multiple choice?
This configuration is not made in the spreadsheet. After importing, go to: Settings > Checklists > Structure and edit the desired item.
